Identity Validation Form

For the safety and security of our community, all individuals attending Friday evening and Saturday morning services, as well as Religious School parents present on Saturdays, must complete this form to validate their identity.

You only need to complete the form once, and your name will be added to our security list for future visits. Please submit one form per person.

Upon arrival, our security volunteers will reference this list to ensure a safe and welcoming environment for all.
